Neuromusculoskeletal pain ranges from mild to severe, from brief to chronic, and from irritating to debilitating. Neuromusculoskeletal problems sometimes manifest as functional impairments rather than pain, but are no less important for the absence of pain.
Physical Therapy properly applied considers not only these variations but also clinical signs and symptoms to identify the pain generating entity, educate the patient about their pain, empower the patient with ways to address symptoms and causes at home and to apply manual techniques, therapeutic exercise and/or modalities to address both the pain generator and symptoms.
Without all of the above components, physical therapy interventions are less likely to succeed.
Identifying the pain generator, sometimes readily palpable or elicited, sometimes rather obscure, is the foundation of quality therapy care. In addition to signs and symptoms, the mechanism of onset, daily activity and pain patterns are often valuable points of information which can only be obtained by carefully listening to the patient. For this reason, we carefully interview our patients at the evaluation.
Successful outcomes in therapy often are heavily dependent on a patient's adherence to a program at home or work. Achieving the patient's confidence that this time and energy commitment is worthwhile is critical for compliance. Time spent clarifying the nature of the problem and the expected impact of the patient's participation on the problem is time well spent.
The patient's efforts at home must often be supported with manual or “hands on” interventions in the clinic. Whether for nerve gliding, myofascial release, proprioceptive re-education, stretching or improving joint mobility by inducing correct arthrokinematics, manual therapy is often critical.
Therapeutic heat and cold, therapeutic ultrasound, electrical stimulation, traction and iontophoresis are used as needed to control inflammation, swelling and pain or to expedite healing by providing biofeedback, inducing muscle contractions or increasing circulation locally in an effort to expedite healing.
